KVFR is committed to fulfilling public records requests in conformance with RCW Ch. 42.56 and Ch. 44-14 WAC. 

Requests must include:

• Contact information of requester, including name and email.
• A description of the incident, including date, address, and individuals involved.
• Signed patient authorization form for medical records in accordance with RCW 70.02.030.
